Step Two
Send supporting documentation to the MIS department

Please remember that, in addition to completing the online application, you are required to send supporting documentation directly to the MIS department.

Make sure your full name is on all of your documents.

All applications must be received by the deadline. Applications will NOT be reviewed or considered until ALL of the following supporting documents are received.

1. Documents from a US institution: Final, official transcript showing degree awarded with conferred date
   International documents: Final, official transcripts/academic records showing degree awarded with conferred date and/or official degree/diploma certificate (Please refer to the Graduate College website for more detailed information:http://grad.arizona.edu/admissions/admissions-requirements/all-degree/transcript-requirements). Documents must be sealed and 2 of each for either domestic or international.
   
2. Two letters of recommendation (submitted online)
   
3. Official GRE or GMAT scores taken within the last five years of intented term of enrollment (sent to us from ETS)
   
4. Copy of GRE or GMAT scores taken within the last five years of intented term of enrollment
   
5. Official TOEFL scores taken within the last two years of intented term of enrollment (sent to us from ETS)
   
6. Copy of TOEFL scores taken within the last two years of intended term of enrollment
   
7. Resume (which includes work experience with dates of employment)

ETS School Codes: When requesting official score reports from ETS, please use the following:

• GRE: 4832 (The University of Arizona)
• GMAT: 4832 (The University of Arizona)

Application Deadlines

The MIS department accepts applications year round. We will review all completed applications. Those received by the deadlines below (or next business day for deadlines falling on weekends or holidays) are given the highest priority. All others will be considered based on space availability.

If your preferred term has reached capacity, you will be automatically considered for the next term. Note that those interested in the Dual Degree program must apply separately to the Eller MBA program.
